✔️ Dosage & Administration

• Can be taken with or without food, but it's ideal to take it at the same time daily to maintain stable drug levels.
• Do not chew or crush; swallow whole.
• If a dose is missed, take it as soon as it is remembered. Do not double dose.
• Do not discontinue abruptly without consulting your doctor—sudden stoppage may worsen symptoms.

Adult Dosing

Schizophrenia: Start with 5–10 mg/day. Titrate gradually if needed. Maintenance: 10–20 mg/day (max: 20 mg).

Bipolar Mania (Monotherapy or with lithium/valproate):

• Monotherapy: 10–15 mg/day initially.
• With lithium/valproate: 10 mg/day initially.
• Maintenance: 5–20 mg/day (max: 20 mg).

Bipolar Depression (with fluoxetine): Start at 5 mg/day, adjustable up to 20 mg/day in resistant cases.

Hepatic or Renal Impairment: Start at 5 mg/day.

Pediatric (13–17 yrs): Start with 2.5–5 mg/day; target dose: 10 mg/day; max: 20 mg/day.

Not recommended for children under 13.

✔️ Side Effects

Common (≥10%)

• Dizziness, drowsiness, weight gain
• Dry mouth, constipation
• Orthostatic hypotension (sudden BP drop when standing)
• Muscle stiffness, restlessness
• Tremor, increased prolactin
• Insomnia, elevated liver enzymes

Less Common (1–10%)

• Tremors, weakness, akathisia, parkinsonism
• Hypotension, postural hypotension

Rare but Serious (<1%)

• Sudden cardiac death, seizures, diabetic ketoacidosis
• Neuroleptic malignant syndrome (NMS)
• Pulmonary embolism, suicidal ideation, pancreatitis
• Tardive dyskinesia

Weight gain, blood sugar, and cholesterol elevation are notable metabolic concerns—lifestyle modification and regular monitoring are strongly advised.

✔️ Patient Guidance

• May cause sleepiness and dizziness—avoid driving until you know your response.
• Monitor weight, blood glucose, and cholesterol levels regularly.
• Do not smoke, as smoking may affect drug levels.
• Inform your doctor if you are pregnant, planning pregnancy, or breastfeeding.
• Stay hydrated and maintain a balanced diet and active lifestyle to minimize side effects.

✔️ Interaction

• May reduce the effectiveness of levodopa and dopamine agonists
• CYP1A2 inducers (e.g., rifampicin, omeprazole) increase clearance
• CYP1A2 inhibitors reduce clearance
• Carbamazepine increases metabolism
• Activated charcoal lowers oral absorption by 50–60%
• Use cautiously with alcohol and CNS depressants

✔️ Contraindications:

• Known allergy to Olanzapine
• Angle-closure glaucoma
• Use in breastfeeding is not recommended
• IM form contraindicated in patients with severe cardiovascular disease or recent heart surgery

✔️ Use with caution in patients with:

• Seizure disorders or risk of seizures
• Blood disorders, liver/kidney impairment
• Diabetes, Parkinson’s disease, prostatic enlargement
• Cardiovascular conditions, cerebrovascular disease
• Elderly patients with dementia-related psychosis
• Those exposed to medications prolonging QT interval or causing CNS depression

Patients should avoid driving or operating machinery until they understand how the drug affects them. Stand slowly from sitting/lying positions to avoid dizziness.
